PPO Plan
The Preferred Provider Organization (PPO) is a plan that provides coverage through a network of contracted health care providers, hospitals, and other medical professionals (preferred providers). By using “in-network” services, plan members benefit from reduced rates that are negotiated by the insurance carrier. Most Boise State employees enroll in the PPO Plan.
- After paying an annual deductible, this plan generally pays eighty percent (80%) of most Allowable Charges provided by an in-network provider.
- In-network Physician Office Visits (office exam only) require a $20 copayment and are not subject to the annual deductible.
- Eligible out-of-network services are subject to a separate deductible and are generally reimbursed at sixty percent (60%) of most Allowable Charges.
Traditional Plan
The Traditional plan allows enrollees to access in-network or out-of-network providers and facilities for higher out of pocket costs. Although enrollees pay higher premiums, deductibles, and co-insurance rates, they enjoy the flexibility of not being restricted by the insurance carrier’s network.
- This plan provides payment of seventy percent (70%) of the Allowable Charges for most covered expenses, after an annual deductible.
High Deductible Health Plan
The High Deductible Health Plan (HDHP) allows enrollees to save money on their monthly premiums. In return, these members pay higher deductibles and out of pocket maximums than other plans. This may be a good option for those who don’t anticipate many medical expenses. The HDHP plan utilizes the same network as the PPO.
- After paying a higher annual deductible, the plan generally pays eighty percent (80%) of most Allowable Charges provided by an In-Network provider.
- Eligible Out-of-Network services are subject to the same deductible but are generally reimbursed at sixty percent (60%) of most Allowable Charges once met.
- A Health Savings Account (HSA) is designed to work with a High Deductible Health Plan (HDHP). It allows you to contribute a portion of your salary pre-tax to cover qualified medical expenses.
Medical Premiums
Employee contributions for medical premiums are deducted from the first two paychecks each month and go towards coverage for the following month (i.e., premiums for January are pulled from December’s paycheck). For information about bi-weekly premiums, please refer to this website.
